salomesvanadze Have you noticed how differently you eat when you’re exhausted?

More hungry. More cravings. Something sweet suddenly sounds much more tempting. And somehow your usual self-control feels much harder.

We talk a lot about nutrition and exercise when we talk about weight. Much less about sleep.

And for young mothers especially, I think we sometimes ask far too much: care for a baby, function on broken sleep, manage everything else… and somehow get your body back at the same time.

So before blaming yourself or rushing to “fix” your body, perhaps ask a different question:

Am I actually getting enough rest to ask all of this from myself?

Sometimes taking care of your body starts with asking it to do less, not more. 🤍

I certainly have.

My six-year-old Max keeps teaching me the difference between guiding someone and quietly writing their story for them.

And we do this with partners, friends, colleagues, even patients.

Sometimes caring for someone means giving them everything you can… and leaving enough space for them to choose for themselves.

Parents, where do you find this hardest?

By hour two, you’ve decided they’re upset with you.

By hour four, you’ve reconstructed your last conversation.

By evening, you’re reconsidering the entire relationship.

And then they text:

“Sorry, crazy day.”

😅

I’m exaggerating. Slightly.

But I think we all do versions of this.

We take something that happened, immediately give it a meaning, and then react to the meaning rather than to what actually happened.

One thing I’m trying to practise more:

What do I actually know?
What am I assuming?
And what else could be true?

Sometimes it IS about you.

Sometimes there is something to learn, apologise for or change.

And sometimes… someone was simply having a crazy day.

Life becomes surprisingly lighter when you learn the difference.

salomesvanadze The “best” incision doesn’t exist.

One of the most common questions I receive is:

“Which incision is the best for breast implants?”

The answer may surprise you.

There isn’t one incision that’s best for everyone.

Every approach has its own advantages, limitations and indications.

If your anatomy allows it, my preferred approach is a short periareolar incision, placed along the lower or higher border of the areola.

It is technically more demanding, requiring precise dissection through a smaller opening, but in carefully selected patients it allows for excellent access while often leaving a very discreet scar that blends naturally with the edge of the areola.

Years ago, there were concerns that this approach might increase the risk of capsular contracture.

With modern surgical techniques, meticulous skin preparation, antibiotic protocols and evidence from more recent studies, this has not been shown to carry a higher risk than other commonly used incisions.

Ultimately, the incision is not chosen because it is my favourite.

It is chosen because it is the right option for that particular patient.

Every operation should be tailored to the individual, not the other way around.

salomesvanadze People often ask me what motivates me after all these years.

The answer has changed.

Early in my career, I wanted to become technically better.

Then I wanted to build something of my own.

Today, what keeps me going is much simpler.

I want every patient who walks through our doors to feel genuinely seen, heard and cared for.

Surgery is simply one of the ways I can express that.

Over the years, I’ve realised that why is not a destination you suddenly discover one morning.

It evolves.

It becomes clearer with experience, with mistakes, with difficult conversations, and with every person whose life you have the privilege to be part of.

For me, knowing why has made decisions easier.

It has helped me say no to things that didn’t align with my values, and yes to the ones that truly mattered.

Your why doesn’t have to be extraordinary.

It just has to be true.
